The device, called the ExAblate Neuro, uses focused ultrasound, pulses of ultrasound energy that can heat up tissue and destroy it. With the accompanying MRI scanner, doctors can guide the ultrasound blast, or sonication, using real-time imaging of brain anatomy, while monitoring the treatments progress.

The U.S. Food and Drug Administration has approved an incisionless form of brain surgery to treat advanced Parkinsons disease after successful testing at the University of Virginia School of Medicine and other sites.
The authorization allows the use of Insightecs Exablate Neuro focused ultrasound device to treat problems with mobility, rigidity and involuntary movements, known as dyskinesias, that are common in Parkinsons. What Neurologic Conditions Can Be Treated With Mr
The U.S. Food and Drug Administration has approved focused ultrasound for the following conditions:
- Essential tremor. MR-guided focused ultrasound is approved for the treatment of essential tremor that cannot be controlled with medication. Approval is for treatment of one side of the brain only. Patients must be at least 22 years of age.
- Tremor-dominant Parkinsons disease. MR-guided focused ultrasound is approved to treat patients with Parkinsons disease whose main symptom is tremor. Patients must be at least 30 years of age.
Use of MR-guided focused ultrasound is being explored for other neurologic conditions including the tremors associated with multiple sclerosis, epileptic seizures that cannot be controlled by other treatment approaches, other movement disorders, stroke, brain tumors and neuropathic pain. Treating these other conditions is considered experimental at this time.
Surgical Procedure For Mrgfus
Conventional MRgFUS was performed at the left posteroventral GPi according to a previously described protocol.13,14 The entire procedure was performed using a 3-T MRI scanner using the Exablate 4000 device . The entire scalp was completely shaved, and a scalp nerve block was performed to prevent the experience of pain during the cooling stage.20 A Cosman-Roberts-Wells stereotactic frame was then fixed on the patients head under local anesthesia. A flexible silicone membrane with a central hole was applied to the patients head to seal the space between the head and the transducer. This space contained cool, degassed water to reduce heating of the scalp. Prior to initiation of treatment, a series of standard diagnostic MRI scans were obtained to identify the location requiring treatment. The target area of the left GPi included the following coordinates: 20 mm lateral to the midline, 34 mm anterior to the midpoint between the anterior and posterior commissures, and 34 mm inferior to the intercommissural line.
