How does a physician arrive at a Parkinson’s disease diagnosis?

There is no lab or imaging test that is recommended or definitive for Parkinsons disease. However, in 2011, the U.S. Food and Drug Administration approved an imaging scan called the DaTscan. This technique allows doctors to see detailed pictures of the brains dopamine system.

A DaTscan involves an injection of a small amount of a radioactive drug and a machine called a single-photon emission computed tomography scanner, similar to an MRI.

The drug binds to dopamine transmitters in the brain, showing where in the brain dopaminergic neurons are.

The results of a DaTscan cant show that you have Parkinsons, but they can help your doctor confirm a diagnosis or rule out a Parkinsons mimic.

How Is Parkinson’s Disease Diagnosed

Your doctor will ask questions about your symptoms and your past health and will do a neurological exam. This exam includes questions and tests that show how well your nerves are working. For example, your doctor will watch how you move. He or she will check your muscle strength and reflexes and will check your vision.

Your doctor also may check your sense of smell and ask you questions about your mood.

In some cases, your doctor will have you try a medicine for Parkinson’s disease. If that medicine helps your symptoms, it may help the doctor find out if you have the disease.

Tests

There are no lab or blood tests that can help your doctor know whether you have Parkinson’s. But you may have tests to help your doctor rule out other diseases that could be causing your symptoms. For example:

  • An MRI or CT scan is used to look for signs of a stroke or brain tumor.
  • Blood tests check for abnormal thyroid hormone levels or liver damage.

Another type of imaging test, called PET, sometimes may detect low levels of dopamine in the brain. These low levels are a key feature of Parkinson’s. But PET scanning isn’t commonly used to evaluate Parkinson’s. That’s because it’s very expensive, not available in many hospitals, and only used experimentally.

Determining Diagnosis Through Response To Parkinsons Medication

If a persons symptoms and neurologic examination are only suggestive of Parkinsons disease or if the diagnosis is otherwise in doubt, the physician may, nevertheless, prescribe a medication intended for Parkinsons disease to provide additional information. In the case of idiopathic Parkinsons, there is typically a positive, predictable response to Parkinsons disease medication in the case of some related Parkinsonian syndromes, the response to medication may not be particularly robust, or it may be absent entirely.

Unfortunately, there are no standard biological tests for the disease, such as a blood test. However, researchers are actively trying to find biomarkers in blood and other bodily fluids that could help confirm the diagnosis.

Elimination Of Other Conditions

Although no test can diagnose Parkinson’s disease itself, your doctor may order blood tests or imaging studies to rule out other causes of your symptoms.

Your doctor will want to know about any medication or recreational drugs you take, since some drugs can cause symptoms similar to those of Parkinson’s.

Parkinson’s disease doesn’t show up on radiological studies like X-ray, MRI , or CT scans.

Doctors sometimes order dopamine transporter SPECT imaging to determine whether a person with suspected Parkinson’s has a different disorder called essential tremor.

Severe Headaches Are A Main Symptom Of Parkinson’s Disease

How Do They Diagnose Parkinson

There are several common symptoms of Parkinson’s disease, though severe headaches are not one of them. PD is diagnosed when a person has one or more of the four most common motor symptoms of the disease that include resting tremor, slow movement , rigidity, and difficulty balancing when standing . There are other secondary motor and non-motor symptoms that also occur with PD. Symptoms may be experienced differently by each person and the progression of the disease is different for everyone as well. For example, some people may have tremor as a primary symptom, while another may not have tremors but may have postural instability.

Physical Examination And Tests

A trip to the neurologists office often includes what seems like dozens of questions, along with multiple tests.

There currently are no diagnostic blood tests for Parkinson’s disease, but your doctor may do some routine blood and urine tests to assess your overall health. Your blood pressure will be taken sitting and standing to look for orthostatic hypotension.

A movement disorder specialist will do a variety of physical tests to assess you as well.

What Causes Parkinson’s Disease

Parkinson’s disease is the result of the loss of the brain chemical dopamine. When nerve cells, called neurons, in an area of the brain that controls movement become impaired and/or die, the amount of dopamine they normally produce decreases. This loss of dopamine causes the movement problems seen in people with PD.

Clinical Basis Of The Levodopa Test

Parkinsons disease is difficult to diagnose, especially in the early stages. Several neurological diseases such as atypical Parkinsonian disorders show similar symptoms to Parkinsons disease, but have different causes. There is no single test that can definitively identify Parkinsons disease, and 5 to 10 percent of patients are misdiagnosed. Up to 20 percent of patients who are diagnosed with Parkinsons disease are later found to have other conditions.

Parkinsons disease is characterized by low levels of a neurotransmitter called dopamine in the brain. This is caused by the death of dopaminergic neurons in a region of the brain called the substantia nigra, which regulates motor functions and normal physical activity. This causes progressive deterioration of motor functions in Parkinsons disease patients resulting in typical motor symptoms .

Levodopa is the precursor of dopamine that, unlike dopamine, can cross the blood-brain barrier. Once inside the brain, it is converted into dopamine by the enzyme, dopa decarboxylase and replenishes dopamine levels, thereby restoring or improving motor function. That is why patient response to levodopa can be used to distinguish Parkinsons disease from other neurological conditions.

Understanding Parkinsons Disease

The diagnosis of Parkinson’s disease relies on expert opinion. Autopsy studies, however, have demonstrated that even experienced neurologists misdiagnose Parkinson’s disease in about a quarter out of a hundred cases. Diagnostic accuracy at disease onset, when neuroprotective treatment is anticipated to be most effective, is even lower. Thus, there is a crucial need for biomarkers that are disease-specific and which precisely identify early disease stages.

Traditional studies of blood from Parkinson’s disease patients have analyzed expression levels of one gene or gene product at a time. We plan to take advantage of ‘gene chip’ technology allowing expression analysis of up to 22,000 genes on a single glass slide, known as microarray. We hypothesize that a comparison of the gene chip analyses of blood samples from Parkinson’s disease patients and normal controls or patients with other neurological diseases will identify a set of signature genes with characteristic expression in patients with Parkinson’s disease. These key genes will provide a ‘molecular fingerprint’ of Parkinson’s disease in blood.

Besides Medication What Other Approaches Should I Use To Deal With This Disease

Medication can certainly reduce the complications of Parkinsons disease, but its not the only way to deal with it. There are other approaches that you may need to consider in order to feel and live much better with this progressive disease. Here are some of them:

  • Educate yourself about the disease

Blood Tests And Spinal Fluid Tests

A blood test or spinal fluid test cant be used to diagnose Parkinsons. But they can be used to search for certain proteins that indicate you may have another neurodegenerative condition with similar symptoms.

The presence of elevated levels of a nerve protein called neurofilament light chain protein may indicate that you have another movement disorder, such as:

  • multiple system atrophy
  • corticobasal degeneration

What Are The Diagnostic Criteria For Parkinsons

Doctors diagnose Parkinsons clinically based on your symptoms and medical history.

No individual test can be used to diagnose Parkinsons. Many other neurogenerative conditions can lead to similar symptoms, so your doctor may use a blood test, brain scans, or other tests to rule out other conditions.

Essential tremor is the most common type of tremor. Its most noticeable when your hands are doing something and it usually affects both the right and left sides of the body equally. Essential tremors often lessen when your body is resting.

Unlike an essential tremor, a Parkinsons tremor is most obvious when the affected body part is resting and tends to be less noticeable with movement. It usually starts on one side of the body and may progress to the other side as Parkinsons develops.

The time it takes to get a diagnosis can vary from person to person. Some people may receive a diagnosis of Parkinsons quite quickly, but for others it may be a long process. This can be due to a number of things, including your medical history, your age and what symptoms you have.

Your specialist may wish to rule out other causes of your symptoms first and see how you respond to treatment. This may take some time, and, as already mentioned, there is currently no definitive test for Parkinsons.

How you respond to treatment may help your specialist make a diagnosis. Keeping a diary or record of your symptoms will give the specialist more information to guide their decision.

Because the symptoms of Parkinsons are sometimes similar to other forms of parkinsonism, people can sometimes be misdiagnosed.

What Is Parkinson’s Disease

Parkinsons disease occurs when brain cells that make dopamine, a chemical that coordinates movement, stop working or die. Because PD can cause tremor, slowness, stiffness, and walking and balance problems, it is called a movement disorder. The Importance Of Early Diagnosis

Early detection and diagnosis is important because the treatments for PD are more effective in the early stages of the disease. In addition, physical therapy and exercise, which greatly improve symptoms and delay progression of the disease, are much easier to perform in the early stages.

Current diagnosis is made through the presence of motor symptoms however, researchers have found that by the time motor symptoms occur, over 60% of all dopamine neurons in the basal ganglia of the brain have been damaged. Non-motor symptoms become apparent in people with PD long before motor symptoms, including sleep disturbances and loss of the sense of smell.3

Active areas of research include looking for markers in the blood, urine, or cerebral spinal fluid that reliably detect PD, called biomarkers. In addition, brain imaging tests that have high sensitivity for detecting PD are also being actively researched.4

Parkinsons Is A Progressive Disease Whats Meant By That

Parkinsons is not like a flu or fever that you take antibiotics and recover in 2-3 days. It is a progressive disease that means it develops and becomes worse over time.

At first, some abnormal changes appear in the brain that begins to damage parts of the brain responsible for movements. These changes are gradual and the effect is limited to the brain only.

After 15-20 years, the damage reaches the level where it begins to affect body normal movements. This is the early stage where typical symptoms of the disease start to develop. These early symptoms are mild and appear either on one or both sides of the body. At this stage, the disease can easily be diagnosed clinically.

After 10 years of diagnosis, the brains ability of performing movement functions is severely affected. The symptoms become severe and begins to affect the patients daily life activities. This is called the mid-stage of the disease.

When the disease has passed 20 years, it reaches its advanced stage. At this stage, the symptoms become very severe and most often the patient needs assistance for mobility.

What Is The Prognosis For Parkinsons Disease

The rate at which Parkinsons progresses varies from patient to patient. Some patients experience its changes over 20 years or more. While others find the disease advances quicker.

Parkinsons is not a fatal disease. However, secondary complications from symptoms may increase falls, blood clots or pneumonia, which can be life-threatening. These are more common in later stages of Parkinsons.

In general, the average life expectancy of Parkinson’s patients is similar to people without the disease.

How It All Fits Together

Diagnosing Parkinsons disease can be tricky. The process relies heavily on your doctors judgment. In addition, the causes and risk factors of Parkinsons are not entirely clear yet, which contributes to the difficulty in diagnosing this condition.

However, there have been efforts to try and detect this disease earlier. For instance, clinicians have started focusing more on prodromal symptoms, which are early symptoms that appear before movement-related difficulties begin.

These symptoms include:

  • Loss of smell, which can sometimes occur years before other symptoms
  • Chronic constipation, without any other explanation
  • Rapid eye movement behavior disorder, which causes sleep disturbances

  • There are many genes that are associated with the development of PD. This list continues to grow as more genes are discovered. Testing of only some of these genes is available in commercial labs.
  • The majority of people with PD, even those with a family history of PD, do not harbor one of these identified abnormal genes. The genetic contribution to PD in these people is yet to be discovered.
  • For a particular gene there may be a number of different mutations associated with disease, some of which are more common than others. Commercial testing may identify only the most common of the mutations, and therefore not capture everyone who carries a disease-causing mutation.
  • Conversely, only particular mutations in a gene may be associated with disease. Commercial testing may identify changes in a gene that may not have clinical consequences. This can be confusing for patients who even after genetic testing may not know whether they harbor a disease-causing mutation.
  • Different mutations can be enriched in different ethnic populations. For example, Ashkenazi Jews and North African Berbers have an increased risk of carrying Leucine rich repeat kinase 2 mutations. Glucocerebrosidase mutation frequency also varies greatly with ethnicity and is also increased among Ashkenazi Jews.
