What Is The Parkinson’s Exercise Framework

Even though the message from research and the experience of people with Parkinson’s is that exercise is good for you, not all professionals have been able to keep up to date about what people with Parkinson’s should be doing and when. A worrying trend is that some professionals can even be a barrier to a person’s progress.

The Parkinson’s exercise framework was developed to help answer questions about what the right type of exercise is for people with Parkinson’s at different stages of the condition.

A group of expert physiotherapists came together to develop the framework based on research and speaking to people with Parkinson’s and professionals. The aim is to provide information on how people with Parkinson’s should be engaging in exercise safely and effectively.

Types Of Exercise Suitable For Parkinsons Disease Patients

If you have Parkinsons disease, there are a lot of health benefits that come along with exercise. Staying active can help you sleep, strengthen your muscles and joints, reduce stress and depression, and improve posture, balance, and gait.

But what sort of exercise should you do? The types of exercise you choose will depend, to some degree, on the severity of your Parkinsons disease and your overall health. According to the Parkinsons Disease Clinic and Research Center at the University of California, the exercises should be varied and incorporate changing directions through unplanned movement, cardiovascular exercise, balance, strength training and rhythmical exercises.

Unplanned and Random MovementThe exercises listed require the person to change tempo and direction regularly. These will challenge a person mentally as well as physically as they require concentration to perform.

Basic Science Supporting Exercise As A Disease Modifier In Pd

In animal models of PD, exercise has shown to be neuroprotective against the neurotoxins, 6-OHDA and MPTP. Several protective mechanisms have been implicated, including neurotrophic growth factors release, anti-oxidation, and anti-inflammation . Tillerson et al. demonstrated in both 6-OHDA and MPTP rodent models that fixed-speed treadmill exercise twice a day for 10 days post-lesioning resulted in recovery of behavioral deficits and attenuated the loss of striatal dopamine, DOPAC, homovanillic acid, dopamine transporter, tyrosine hydroxylase, and vesicular monoamine transporter compared to those rodents who were not exposed to exercise. A prior study by Tillerson et al. also showed that forcing unilaterally 6-OHDA-lesioned mice to use their contralateral, impaired forelimb for the first 7 days post-intrastriatal 6-OHDA infusion could also attenuate both the resulting neurochemical as well as behavioral deficits. They postulated that this neuroprotection was due to the release of neurotrophic growth factors, and then provided evidence for glia cell-derived neurotrophic factor as a candidate mediator .

Fitness Tips To Manage Pd Symptoms

  • Choose an exercise program that you will actually do! Dont design a great, Parkinsons-specific program and then skip it because its too hard or not fun.
  • Follow a varied routine. Perform simple stretches and posture exercises daily, and make sure to include aerobic and strengthening exercises several times per week.

A rating of perceived exertion is a good way to measure intensity. On a scale from 0 to 10, 0 would be how you feel while sitting or lying down, while 10 would be the maximum effort you can give. Building up to an effort between 5 to 8 means you are exercising at a high intensity. A good gauge is, if you can have a conversation with someone while exercising, you should probably increase your intensity.

How Can I Benefit From Exercise

Symptom Management

Research has shown that exercise can improve gait, balance, tremor, flexibility, grip strength and motor coordination. Exercise such as treadmill training and biking have all been shown to benefit, along with Tai Chi and yoga. So far, studies have shown:

  • Engaging in any level of physical activity is beneficial, rather than being sedentary this is associated with improved motor symptoms.
  • For people with mild to moderate PD, targeted exercises can address specific symptoms for example: aerobic exercise improves fitness, walking exercises assist in gait, resistance training strengthens muscles. One study showed that twice-a-week tango dancing classes helped people with PD improve motor symptoms, balance and walking speed.
  • Exercise may also improve cognition, depression and fatigue, but the research is still ongoing in these areas.

One study showed that people with PD who exercised regularly for 2.5 hours a week had a smaller decline in mobility and quality of life over two years. Research is ongoing to discover therapies that will change the course of the disease.

Neurologists within the Parkinsons Foundation Center of Excellence network recommend a regimented exercise program to their patients and also to people who are worried about getting PD due to family connection.

How Can Exercise Help People With Parkinson’s

Exercise good for people with Parkinson’s and their general wellbeing. But we now know that the right type and intensity of exercise can actually slow the progression of Parkinson’s symptoms.

The right exercise can help people remain physically active, and reduce discomfort from pain and other symptoms affecting mobility. Exercise done in a group setting can contribute to a sociable and active lifestyle.

Exercise also improves sleep. It can even help with issues that many people don’t talk openly about, such as constipation or problems with mood.

Many people with Parkinson’s also talk of exercise as a way of taking back some control in order to manage their symptoms, with some going as far as to say it helps them “fight back” against the condition.

Chasing Protection In Parkinsons Disease: Does Exercise Reduce Risk And Progression

Exercise may be the most commonly offered yet least consistently followed therapeutic advice for people with Parkinsons disease . Epidemiological studies of prospectively followed cohorts have shown a lower risk for later developing PD in healthy people who report moderate to high levels of physical activity, and slower rates of motor and non-motor symptom progression in people with PD who report higher baseline physical activity. In animal models of PD, exercise can reduce inflammation, decrease -synuclein expression, reduce mitochondrial dysfunction, and increase neurotrophic growth factor expression. Randomized controlled trials of exercise in PD have provided clear evidence for short-term benefits on many PD measurements scales, ranging from disease severity to quality of life. In this review, we present these convergent epidemiological and laboratory data with particular attention to translationally relevant features of exercise . In the context of these findings we will discuss clinical trial experience, design challenges, and emerging opportunities for determining whether exercise can prevent PD or slow its long-term progression.

What Is The Best Type Of Exercise For Parkinson’s

As Parkinson’s affects everyone differently, there is no ideal solution or exercise for everyone. So the exercise framework suggests a blend of styles and intensity that will help people with Parkinson’s do what they can at different times over the course of their condition.

Some people should be participating in more vigorous exercise at the gym, or out cycling and running with friends. Others are best doing chair-based exercises at home.

Pathophysiology Of Parkinsons Disease

The progressive death of dopaminergic neurons in the Substantia Nigra pars compacta located in the midbrain, more precisely in the Basal Ganglia , promotes a significant decrease in the levels of the neurotransmitter dopamine, and as a consequence the functional impairment of the neural circuits . The chronic reduction in dopamine levels gives rise to the manifestation of the motor symptoms that characterize this disease. However, the pathophysiology of PD is not limited to the dopaminergic system, that is, neuronal degeneration of other areas of the brain such as the brainstem and cortex competes and even precedes neuronal death in BG. Thus, other neurotransmitter systems are compromised, thus making PD a multisystemic pathology manifested by a series of motor and non-motor symptoms .

The cardinal motor signs of PD are: resting tremor, plastic-type muscular rigidity, bradykinesia , and postural instability.
