I recently heard that pesticides cause Parkinsons disease. If this is true, can you tell me whether buying organic fruits and vegetables provides adequate protection? My mother had Parkinsons so I worry about this disease a lot.

Parkinsons disease is the second most common neurodegenerative disorder worldwide behind Alzheimers disease. Parkinsons affects nerve cells in the part of the mid-brain known as the substantia nigra, responsible for muscle movement. Symptoms include tremors, rigidity, slow movements and balance problems that worsen over time. Parkinson patients often exhibit signs of depression and may eventually develop cognitive problems, including dementia. But although PD may result in disability, the disease often moves slowly, and most people have a number of years of only minor disability following a diagnosis.

For some time, experts have believed that unusual exposure to herbicides and pesticides increase the risk of developing Parkinsons disease. The latest word on this subject comes from Italian researchers who reviewed 104 studies from around the world and concluded that exposure to pesticides, bug and weed killers and solvents appears to be associated with a 33 to 80 percent increase in the risk of developing PD. The investigators also reported that in controlled studies, exposure to specific chemical compounds including paraquat, a weed killer, and two fungicides, maneb and mancozeb, doubled the risk.

The Ghosts Of Pesticides Past

Over the past half century, we have begun to identify the worst risks and address them. The insecticide DDT was once considered a miracle compound. In the 1930s, the Swiss chemist Paul Hermann Müller was looking for a chemical that could kill insects that were destroying crops and spreading diseasewithout harming the plants. Müller, a nature lover, tested hundreds of chemicals before coating the inside of a glass box with DDT, a colorless, tasteless, and almost odorless nerve toxin. He placed houseflies into the container, and they bit the dust. Müller had found his answer.

In the 1940s DDT was considered harmless to humans and regularly sprayed on neighborhoods here children play in the sprays at the beach in New York. Even though DDT was banned half a century ago, it persists in the environmentand in our food supply. It becomes more concentrated as it makes its way up the chain to human consumption. The pesticide is then stored in fatty tissues, including the brain. DDT has been linked to Parkinsons disease. Because of the widespread use of the pesticide, DDT and its breakdown products are detectable in nearly everyone in the United States. It has also been found in the breast milk of women living in Spain, Nicaragua, Taiwan, and the Spanish Canary Islands as recently as 2014.

We eat fruits, vegetables, nuts, and grains every day that have been doused in pesticides. What kind of risk are we all exposed to? We do not know.

    How Do We Know That Paraquat Can Increase The Risk Of Developing Parkinsons Disease

    Extensive scientific research over the last twenty years has demonstrated a link between Parkinsons disease and people who were exposed to Paraquat. In fact, Paraquat use has long been restricted to certified applicators due to concerns based on this research.

    In the last ten years, research on Parkinsons disease has intensified, with two particularly convincing studies involving humans: one by the National Institutes of Health , and one Italian meta-analysis.     Paraquat Linked To Parkinsons

    A 2013 study from UCLA confirmed that exposure to the herbicide Paraquat is linked with a heightened risk of Parkinsons disease. This combines with other research finding that herbicides and pesticides increase the risk of Parkinsons.

    The researchers, from UCLAs Fielding School of Public Health, studied 357 Parkinsons disease cases along with 754 control subjects adults from Central California. The researchers determined increased exposure to the herbicide Paraquat through geographic mapping of their home addresses, together with agricultural use of the chemical on nearby farms. The research found that those living closer to farms that sprayed the herbicide were found to have a 36% increased risk of Parkinsons.

    However, those who experienced a head injury combined with increased Paraquat exposure tripled their chances of having Parkinsons disease.

    Researchers from Mexicos Unidad de Medicina Familiar also studied cases of Parkinsons together with exposure to the herbicide Paraquat among Mexican workers. They also found a positive association between exposure to this chemical and Parkinsons disease.

    A Discussion Of How Environmental Factors Such As Pesticides May Affect Your Risk Of Parkinsons Disease

    During my recent interview on Wisconsin Public Radio, many of the callers asked questions related to environmental risks of Parkinsons disease , specifically, exposures related to farming. Those calls prompted me to delve further into this complicated and murky topic.

    Before we start discussing specific factors in the environment that may increase risk of PD, lets understand some basic ground rules that will help put this topic in perspective

    With that background, let us begin.

    One Of The First Studies To Look At Human Cells

    Paraquat Herbicide Reapproved Despite Links to Parkinsons Disease

    The researchers used stem cells from patients with Parkinsons disease who had a mutation in the gene responsible for encoding the -synuclein protein.

    At least 30 alterations in this gene have been associated with Parkinsons, and -synuclein protein clumps are a well-documented, albeit poorly understood, hallmark of the disease.

    For the new research, the scientists also worked with normal embryonic cells that they modified using genetic editing to replicate the -synuclein genetic mutation.

    Prof. Ryan explains why using human cells makes this study particularly valuable. Until now, he says, the link between pesticides and Parkinsons disease was based primarily on animal studies as well as epidemiological research that demonstrated an increased risk among farmers and others exposed to agricultural chemicals.

    We are one of the first to investigate what is happening inside human cells, explains Prof. Ryan.

    Stem cells are undifferentiated cells that go on to individualize into specific types of cells. Prof. Ryan and his colleagues used the two types of stem cells to derive dopamine-producing nerve cells from them.

    Then, they exposed these dopaminergic neurons which are known to be affected the most by Parkinsons disease to the two pesticides.

    Data Synthesis And Statistical Analysis

    Meta-analysis of results will be considered by investigating heterogeneity among animal and human studies, separately. It is common, though, that environmental health studies have some differences regarding outcome assessments and exposure definitions that could be an obstacle to formal statistical meta-analysis . The heterogeneity associated with pooled effect estimates will be assessed with the use of a 2 test and the I2 statistic . Heterogeneity will be classified as follows: 0 to 40% 30 to 60% 50 to 90% and 75 to 100% .

    Statistical analysis will be conducted using the Comprehensive Meta-Analysis STATA software . If considerable heterogeneity is detected among studies, meta-analysis will not be indicated and results will only be presented in tables or in a narrative synthesis. However, if heterogeneity does not exceed 75%, we will use random effects meta-analysis , which is a more conservative approach of pooling the results. In this case, the measure of association will be presented as odds ratios and mean difference, both with a 95% confidence interval , for studies with dichotomous and continuous data, respectively .

    Does Exposure To Pesticides Herbicides Or Other Pollutants Increase The Risk Of Developing Parkinson Disease

    A meta-analysis of 89 studies, including 6 prospective and 83 case-control studies, found that exposure to pesticides may increase the risk for PD by as much as 80%. Exposure to the weed killer paraquat or to the fungicides maneb or mancozeb is particularly toxic, increasing the risk for PD about 2-fold. Many of the agents studied are no longer used in the United States and Europe however, some are still found in developing parts of the world.

    In case-control studies, PD was associated with exposure to any type of pesticide, herbicide, insecticide, and solvent, with risks ranging from 33% to 80%. Increased PD risk was also associated with proxy conditions of exposure to organic pollutants, such as farming, well-water drinking, and rural living. In addition, risk seemed to increase with length of exposure.

      In a nutshell, paraquat is a highly toxic chemical that is commonly used as a weed killer and for general grass control. Paraquat was first introduced to the general population in 1961, and since then, it became a widely used herbicide throughout the United States and the world.

      No One Definitive Cause Of Parkinsons

      There are no biomarkers or objective screening tests that indicate one has Parkinsons. That said, medical experts have shown that a constellation of factors are linked to it.

      Parkinsons causes are likely a blend of genetics and environmental or other unknown factors. About 10 to 20 percent of Parkinsons disease cases are linked to a genetic cause, says Ted Dawson, M.D., Ph.D., director of the Institute for Cell Engineering at Johns Hopkins. The types are either autosomal dominant or autosomal recessive .

      But that leaves the majority of Parkinsons cases as idiopathic, which means unknown. We think its probably a combination of environmental exposure to toxins or pesticides and your genetic makeup, says Dawson.

      Age. The biggest risk factor for developing Parkinsons is advancing age. The average age of onset is 60.

      Gender. Men are more likely to develop Parkinsons disease than women.

      Genetics. Individuals with a parent or sibling who is affected have approximately two times the chance of developing Parkinsons. Theres been an enormous amount of new information about genetics and new genes identified over the past 10 or 15 years that have opened up a greater understanding of the disease, says Dawson.

      Translating Certainty Ratings Into Levels Of Evidence For Parkinsonian

      Five descriptors will be used to rate the level of evidence: high,moderate,low,inadequate evidence, and evidence of no health effect. The first three descriptors used in the previous step to indicate the certainty of the evidence will be directly converted into levels of evidence. However, if the level of certainty is very low or no evidence is identified, the level of evidence will be considered inadequate .

      The descriptor evidence of no health effect that indicates that paraquat is not related to PD in humans or in rodent models will be considered only when the level of certainty is high .
