Understandably, many people ask for “the best” kind of exercise to help treat Parkinson’s disease. Some people swear by dance classes or boxing. Others find tai chi and yoga helps their balance. Through our grassroots fundraising community Team Fox, people with Parkinson’s run marathons or bike for their health and to raise money for research.

The best exercise is the one that your care team approves and that appeals to you, because you’ll stick with it. Your exercise routine will vary depending on your overall fitness level, but a good first step is to talk to your physician and have a thorough checkup before starting any activity. If your doctor agrees, one good way to start is with a physical therapist. This way, you can get an “exercise prescription” and work with an expert to determine what you can do safely.

How Do I Get Started

It depends on your overall fitness level, but a good first step is to talk to your doctor and have a thorough checkup before starting any activity. For many people, its important to start slowly, and one good way to start is with a physical therapist. This way you can get an exercise prescription and work with an expert to determine what you can do safely. Especially if you havent been regularly exercising, it may be best to begin under the supervision of a professional who has access to professional equipment.

The best way to achieve benefits is to exercise on a consistent basis. People with Parkinsons enrolled in exercise programs with durations longer than six months, regardless of exercise intensity, have shown significant gains in functional balance and mobility as compared to programs of only two-week or ten-week durations.

People new to exercise programs are generally best off working with an individual or group training leader for people whose mobility is significantly affected by PD, a physical therapist may be the best choice for helping to start a program.

Taking part in organised events is a great way to get involved while also raising much-needed funds for Parkinsons research. While many of these are on hold due to the Pandemic some are still running virtually or you can create your own exercise challenge.

Selection Criteria For Studies

Strength training was defined as an intervention in which participants exercised a muscle or group of muscles against an external resistance. For this study, we considered as external resistance cycle ergometer, weight machine, elastic band, punching bag, and water. In further analysis, we included articles in which the effect of strength training in subjects affected by PD was evaluated and the articles that matched the following inclusion criteria based on PICO principles:

  • randomized controlled trials related to both sexes

  • stages 13 on the Hoehn and Yahr scale

  • study design comparing the effects of strength training versus different exercise protocol

  • study outcomes: muscle strength, physical performance, quality of life

  • training/assessment of subjects during the on medication period

  • articles written in English.

Exclusion criteria were as follows:

  • observational studies

  • studies with healthy or non-exercise controls

  • studies employing supplementary intervention therapies in addition to strength training

  • studies with tailored exercise programs to meet individual capacity.

Two authors independently screened the articles by title and abstract against the selection criteria. Articles that were unclear from their title or abstract were reviewed against the selection criteria through the full text. Any discrepancies between authors were resolved through discussion. The second step was to screen all full-text articles that passed the first step.

What Exercise Is Good For Parkinson Disease

Okay, so we can understand why you might not lean toward weightlifting if you have Parkinson’s. But trust us when we tell you that it could be one of the most helpful ways to work out with your condition. Not only does resistance training help to keep your muscles toned and healthy, but it may also reduce the severity of tremors, according to research presented to the American Academy of Neurology. When participants in the research did twice-weekly resistance exercises for an hour at a time, they exhibited less noticeable tremors, with results being seen in as little as six months. “Our results suggest that long-term weight training could be considered by patients and doctors as an important component in managing Parkinson’s disease,” study author Daniel Corcos stated.

If you’re weight training, however, safety is essential. Make sure that you begin slowly — you may want to ease in using light weights and fewer reps — and nail down your form first before advancing to the heavier stuff, says the Parkinson’s Foundation. If you’re working out in a gym, you might find it helpful to write down the exercises you’re planning on doing beforehand, so you don’t omit any of them. Hydration is essential, too, even when you’re not sweating as heavily as you might during a cardio workout, so keep a bottle of water handy.

Dancing Can Help Coordination

If all else fails, dance. No, seriously. This isn’t just a motto that people can use in their everyday lives, it applies when it comes to exercise, too. If you have Parkinson’s and you’ve tried everything else, dancing might be just the ticket. Dancing as exercise can have numerous benefits for people with Parkinson’s, both mental and physical, as research published in Frontiers in Public Health discusses. Dancing, particularly ballroom dancing, was found in a review published in The Arts in Psychotherapy to improve coordination and augment balance. Regular dancing may also provide benefits to individuals who have depression, a psychological symptom of Parkinson’s.

So what type of dance is best for you? Luckily, there are loads to choose from. Aside from ballroom dancing, tap dance, ballet, and even seated dance are all viable options, per Parkinson’s UK. Having said that, virtually any type of dance that you’re comfortable doing with your condition will likely benefit you. What Is Parkinsons Disease

Parkinsons disease is a neurological disorder that causes problems with movement. These can include shaking, stiffness, and issues with walking, balance, and coordination.

Parkinsons disease can also lead to speech problems and side effects including depression, pain, and difficulty sleeping.

Parkinsons disease affects the cells of the area of the midbrain known as the substantia nigra. This part of the brain produces dopamine and relates to reward and movement. Parkinsons causes cells in the substantia nigra to die, which reduces dopamine levels and causes the onset of symptoms.

There are several types of exercises you can do to manage Parkinsons disease. You can create a varied routine based on your specific concerns, fitness level, and overall health.

The Parkinsons-specific exercises are designed to retrain the mind and body. Many participants report a regained ability to successfully manage the disease, improve their quality of life and take back control over their own body movements. Facial Exercises Are Important For Expressiveness

While exercises that benefit your entire body are incredibly useful for people with Parkinson’s, it’s also important to get a little more specific. Facial exercises may be highly beneficial for those with Parkinson’s to combat facial rigidity and “masking,” a loss of expressiveness in the face seen in people with the disease, as the National Institute of Neurological Disorders and Stroke says. Exercising your face may also assist mobility around motor functions like swallowing, and keep your speech clear, says the Cleveland Clinic.

Luckily, too, these exercises are easy to do at home, and often while you’re doing another activity. When you’re eating, for example, try and exaggerate your chewing, making bigger movements with your jaw and chewing for longer. You can do the same thing while you’re speaking, too, opening your mouth wide and being intentionally more expressive with your face. If you’re a reader, saying what you’re reading out loud can be a good way to keep on top of your speech. And if you thought the days of making faces in the mirror were left in your childhood, now is the perfect time to bring them back, with exaggerated expressions helping to keep your face mobile.

Why Is Swallowing Affected In Parkinsons Disease

Generally, we all swallow more than 500 times a day and usually clear our mouths subconsciously.

The rigidity of PD affects the muscles involved in our automatic swallow and the muscles of the jaw and face may be weakened. This results in a reduction in swallowing and affects the control a person may have over their teeth. The weakened muscles may also reduce the tightness that a person achieves in closing their lips. Everyone has difculty swallowing if they cannot close their lips tightly.

If a person cannot chew their food adequately it can also cause problems. Some particles of food are swallowed easily but some may remain in the mouth. This is called piecemeal swallowing and is a common feature of PD. People are unaware that they have more food to swallow and as a result may choke. Food remaining in the mouth can harbour infections.

Often the tongue bunches up in PD and this can cause food to be pushed out through the teeth instead of controlled down the throat.

PD may also affect the muscles involved in carrying the food down into the stomach and make the process much slower.

It can also make the person feel full up, as the food passing down the oesophagus is moving so slowly.

What Is This Telling Us Relevant To Pd

What Exercise Is Good For Parkinson Disease

In the aggregate, these findings do not prove that exercise slows PD progression, but a neuroprotective effect is certainly plausible, if not compelling. Ideally, this would be assessed in a prospective clinical trial, with patients with PD randomized to regular aerobic exercise vs a passive intervention. However, this is challenging because of practical issues. First, variations in PD drug therapy and exercise compliance would tend to confound the outcomes. Second, PD progression is slow and patients would need to be followed for long durations, with potential for substantial dropouts. Third, we have no reliable biomarker of PD progression and would have to rely on indirect indices. Hence, we currently are primarily left with indirect evidence, as summarized above. Despite these challenges, clinical trials directed at chronic vigorous exercise as a treatment strategy deserve serious consideration.

Are There Any Risks Of Exercising With Parkinsons Disease

Some symptoms, like Parkinsons tremors, may seem worse during exercise. But exercise generally improves tremors and other symptoms in the long run.

Reduce challenges by stretching before and after exercise. Use good form to prevent injury. And avoid slippery floors, poor lighting and tripping hazards. If you have pain, stop and rest.

Pushing yourself too hard during exercise can lead to injury. Start slowly and increase intensity and duration over time. Keep a log to track your exercise choices and how you feel. Eventually, youll learn what works best for you.

Effects Of Physical Exercise On The Treatment Of Parkinsons Disease

Other therapeutic strategies have been evaluated clinically and scientifically in recent years in the search for an action to reduce clinical problems of PD, such as, non-pharmacological interventions like physiotherapy and physical exercise . Rehabilitation through physical therapy has a variety of goals and methods that generally promote benefits in parkinsonian mobility, posture, and balance. However, some limitations have been observed in a consensual way by some researchers in two topics: in relation to the benefits that seem to be more immediate , and the variety and low methodological quality of the studies . Other nonpharmacological approaches to rehabilitation in Parkinsonâs disease are the practice of different modalities of physical exercises such as walking, running, strength training, whole body vibration and functional exercises, which are related to the reduction in the risk of falls, decreased motor symptoms, motor performance improvements, balance and gait improvements, positive repercussions in quality of life and executive functions .

Kurtais et al. investigated the effects of six weeks of supervised treadmill walking, three times a week for 40 minutes in patients with mild to moderate PD, and observed significant improvements in lower limb functional parameters such as walking, balance, and agility, and in related parameters, the adaptations promoted by aerobic exercise as increase of peak VO2 and caloric expenditure in METs .

Brain MRI volumetric measurements have revealed progressive brain atrophy in AD, especially affecting the temporal lobe. However, this is attenuated with maintained physical fitness, documented by peak VO2 values during exercise testing. Thus, in patients with AD, peak VO2 correlated with whole brain, white matter, and temporal lobe volumes.42,43 Parenthetically, in a transgenic mouse model of AD, 5 months of exercise significantly reduced brain -amyloid concentrations.44

How To Start Exercising If Youre Living With Parkinsons

Safety is key. The first thing you need to do is talk with your neurologist and primary care doctor to make sure that the exercise regimen that you embark upon is safe for you.

Next, ask for a referral for physical therapy. A physical therapist will be able to figure out what movement challenges you may have and design a program to help you improve. There are certain physical therapists with additional training in Parkinsons. Your physical therapist will work with you for your allotted sessions, and then can help you plan your ongoing exercise regimen that is tailored to you. Additionally, physical therapy can help counteract the tendency for people with PD to reduce the size of their movements.
