Clinical Use Of Melatonin In Parkinsons Disease And Rbd

A phase advance in nocturnal melatonin secretion was observed in PD patients treated with l-DOPA . Patients treated with l-DOPA showed increased melatonin secretion during the day, perhaps as an adaptive response to neurodegeneration . PD patients, particularly those showing excessive daytime sleepiness, exhibited a reduced amplitude of the circulating melatonin rhythm . A chronobiologic therapy for non-motor manifestations of PD, comprising the scheduled exposure to bright light in the morning and melatonin administration at sleep time, can thus be recommended.

The postulation of an association between motor fluctuations in PD and the diurnal variation in circulating melatonin levels relies on the existence of melatonin effects on striatal DA and serotonin content . In view that l-DOPA-related motor complications is seen in about 50% of PD patients after 5°years of treatment, the results obtained in experimental parkinsonism warrant the use of melatonin to decrease l-DOPA doses in PD .

Guide To Parkinsons Disease And Sleep

Parkinsons disease is caused by a complex interaction of environmental and genetic factors.

It is the second most common degenerative disorder and affects 2% of people over the age of 65 years.

It is a progressive disorder that worsens with time.

The cardinal symptoms of PD are due to the loss of dopaminergic neurons in specific areas of the brain.

These include slow movements, tremors, muscle stiffness, and balance problems.

Patients with PD also have non-motor symptoms.

These include smell and taste problems, pain, fatigue, bladder and bowel symptoms, restless leg syndrome, sleep disorders, swallowing difficulty, excessive salivation, double vision, speech problems, mood symptoms, and postural drop of blood pressure.

The diagnosis of PD is clinical and no tests offer absolute diagnostic certainty.

The diagnosis is especially difficult in the initial stage of the disease.

MRI and DAT scans are often used to differentiate PD from other disorders.

Physical Condition Assessment Results

Dr. Stanley Fahn – Myths and Mistakes in PD Treatment – Parkinson Symposium 2016

The data provided by the digital dynamometer were used to estimate the condition of the participants lower limbs. Table 2 summarizes the strength values for the hip and knee of each group . The dominant side of both groups was the right side, allowing a direct comparison between them.

Table 2. Force data obtained from the lower limb strength of the participants.

Regarding hip flexion and extension, Table 2 shows how the group of older adults with Parkinson presented better results. The MJF group presented a reduction of 25.7% and 17.34% compared to the PK group during flexion tests. Besides, significant differences were found between both groups . In extension, the strength values in both the right and left were more remarkable for the PK group. Although this parameter presented significant differences, there is not a great discrepancy in the strength measurements for the left hemisphere since the Parkinson group presented a mean of 127.98 N and the group of older adults presented a mean of 126.62 N.

Concerning the movements of abduction and adduction, both parameters presented statistically significant differences. On the right side, the PK group showed an increase compared to the MJF group of 18.38% in abduction, while in adduction, such an increase was 23.37%. The difference between the groups was considerably more significant for the left side since, for both parameters, the increase exceeded 25%.

BMC shows that the prevalence of freezing of gait in Parkinsons patient stands at 15%.

Gait freezing is a brief moment of lack of forward moment even when you want to move on. It occurs in a Parkinsons patient when they face an obstacle, move to a new pattern on the floor or are negotiating a corner or walking through a narrow hallway.

According to NCBI, assisted walking either on a standard or wheeled walker may increase confidence in a Parkinsons patient. However, it does not increase speed or reduce freezing .

Other than using a walker, famous physiotherapists who run this Youtube channel feel that having rhythmic music or rhythmic counting could help in reducing gait freezing. This could explain why some walkers have beepers that prompt a patient to make another step. Prompting a walker on where next to step on could also help and that is why some walkers have laser attachments. Home exercises might help

According to the research done in 19 PD patients, the wheeled walker was the most recommended. It gets even better if you are to use one that has laser attachment. Though it will not increase the speed of the patient or reduce freezing, it does boost the confidence of the person and is way faster than when using a standard walker.

Another NCBI study found out that the four wheeled walker offers better mobility and safety.

The U-Step walker 2 for Parkinsons is a very stable walker with the option of a laser light designed for Parkinsons patients and other neurological conditions to help overcome freezing episodes.

The U-Step walker is unlike most other walkers, instead, the U-Step 2 surrounds you and moves with you. You will feel as stable as you would feel holding onto another persons arm.

BRAKING SYSTEM The unique braking system is easy to use and puts you in control. The U-Step 2 will not roll unless you are ready to walk. When you lightly squeeze either brake release lever, the unit will roll with you. Once you release the lever, the unit will stop immediately. This feature is particularly helpful when standing up from a chair because the unit will not roll away from you. Reversed braking system is much safer than a traditional walker.

Rolling Resistance Control The rolling speed is adjustable on new U-Step 2. Some people find that wheeled walkers roll too fast for them. That is why we engineered a control for setting the rolling resistance. Loosen the set screw and rotate the adjustment lever to the desired amount of resistance, then re-tighten the set screw.

Spring-Loaded Front Wheel The spring-loaded front caster wheel glides easily over uneven surfaces, such as indoor molding strips and cracks on the footpath. This avoids sudden jolts as well as the need to lift the walker over small obstacles.
