However, subsequent studies indicated that the presence of ET significantly increased the risk of conversion to PD.

In light of these emerging studies, some scholars have sought to classify the coexistence of both conditions as Essential tremor-Parkinson disease syndrome, which is characterized by patients with a long-lasting history of ET that then developed into PD. For those with ET-PD syndrome, research has indicated that patients are older at onset of PD diagnosis, exhibit less severe PD, and are on a lower dosage of levodopa compared with patients with PD without preexisting ET.

What Research Is Being Done

The mission of the National Institute of Neurological Disorders and Stroke is to seek fundamental knowledge about the brain and nervous system and to use that knowledge to reduce the burden of neurological disease. Researchers are working to better understand the underlying brain functions that cause tremor, identify the genetic factors that make individuals more susceptible to the disorder, and develop new and better treatment options.

Brain functioningIt can be difficult to distinguish between movement disorders such as Parkinson’s disease and essential tremor. These debilitating movement disorders have different prognoses and can respond very differently to available therapies. NINDS researchers are working to identify structural and functional changes in the brain using non-invasive neuroimaging techniques to develop sensitive and specific markers for each of these diseases and then track how they change as each disease progresses.

Other researchers are using functional magnetic resonance imaging technology to better understand normal and diseased brain circuit functions and associated motor behaviors. Scientists hope to design therapies that can restore normal brain circuit function in diseases such as Parkinson’s disease and tremor.

How Does Essential Tremor Develop

Essential tremor symptoms often first appear in a person’s 40s, when delicate movements such as threading a needle become difficult. But it’s usually not until people reach their 50s or 60s that the tremor becomes troublesome for example, making it difficult to use a fork, drink from a cup, or write a letter. Essential tremor progresses slowly. Though essential tremor may eventually affect the voice and head, many people find that their symptoms don’t progress beyond mild hand and arm tremor. Tremors are usually absent during sleep.

The tremor in PD usually starts on one side of the body and may develop on the other side as the disease progresses. In ET, the tremor usually affects both sides from the beginning of the condition.2

Many people worry when they have a tremor that it may be a sign of Parkinson’s disease. However, there is another more common cause of tremor known as essential tremor , and it doesn’t have the serious implications of Parkinson’s. In fact, ET is more than ten times as common as Parkinson’s disease, and, unlike Parkinson’s, does not cause worsening unsteadiness, rigidity, or dementia.

Although it might be a relief to hear that your tremor isn’t caused by Parkinson’s disease, it doesn’t necessarily make the tremor less annoying. For some people, essential tremor comes and goes and hardly bothers them. For others, essential tremor is so bad that tying shoes or eating with utensils is nearly impossible.

What Are The Symptoms Of Tremor

Symptoms of tremor may include:

Treatment Of Essential Tremor

There is no cure for essential tremor. Treatment aims to suppress the involuntary movements. To identify the best treatment, you should consult with a Neurologist. Treatments can include:

  • avoidance of known triggers, such as alcohol or caffeine
  • some medications used to treat other medical conditions, such as heart disease and epilepsy these medications have been helpful in some cases
  • other medications, including tranquillisers
  • deep brain stimulation, a surgically implanted device that helps to short-circuit the impulses or brain waves causing the tremor
  • brain surgery to alter the part of the brain causing the tremor has been successful in some cases
  • MRI guided focused ultrasound
  • regular monitoring and observation if the tremor is mild and doesnt stop the person from performing their usual activities, this may be the only treatment.

In some cases, alcohol may reduce the tremors. However, this is not a recommended treatment, because long-term consumption of alcohol has significant health risks.

Recommendation For Those With Et

ET, PD and parkinsonism have one thing in common: they dont affect individuals in identical ways. There are accepted diagnostic guidelines, but an infinite number of small or large differences can occur: rate of progression, response to medication, nature of lifestyle problems and impairment, etc. If two persons with ET were to compare notes, they would quickly find that things like propranolol helps one but not the other, or one of them gets terrible restless leg syndrome at night while the other has never had it, etc. Thus, there is a growing theory that ET is not a single disorder, but a group of disorders.

For those with ET who notice that they have additional symptoms that dont fit the classic ET descriptions in books or websites, we recommend discussing the situation with a movement disorder specialist. Perhaps theres no cause for concern, but it may be worth seeking an evaluation to rule out a co-occurring form of parkinsonism. Accurate information may be your best friend, since it both equips you and your doctor to strategize treatment, and also to manage anxiety and gain peace of mind.

Such tests include blood and tissue analysis that may reveal distinct cellular changes or biomarkers, and various types of imaging for abnormalities . Also, many diseases lead to death if left untreated, or if there is no known cure. Thus, ET does not qualify as a diseasebut this is of little comfort if tremors become so severe that life is no longer normal.

However, ET is sometimes incorrectly diagnosed as Parkinsons disease . One of the main symptoms of PD is tremors, usually starting in the fingers or hands, but which can also affect other parts of the body as the disease progresses. In addition, PD brings stiffness and rigidity, slowness of voluntary movements, facial and postural drooping, balance and gait problems, skin abnormalities, cognitive and emotional difficulties, and eventually near-total disability. Thus, a diagnosis of PD is devastating.

Tremor In Other Conditions

While tremor is a common symptom of Parkinsons, it can also be a symptom of other conditions, most notably essential tremor. The main difference between Parkinsons tremor and most other types of tremor is that in Parkinsons resting tremor is most common. Other conditions are usually characterized by action tremor, which tends to lessen at rest and increase when youre doing something, like trying to make a phone call or take a drink.

Tremors of the head and voice are also common in essential tremor but rare in Parkinsons.

Are There Surgical Options For Essential Tremor

Deep brain stimulation is a therapeutic option for those with severe disabling ET not already managed by medications. DBS is often described as a pacemaker for the brain. It works much like a pacemaker, sending electrical signals to the brain instead of the heart. It is primarily utilized for patients who have Parkinsons disease, dystonia, or essential tremor, who cant adequately control their disease with medication.

Thalamotomy, involving the destruction of tremor producing cells in the brain region called the thalamus, is another surgical option. Using a small temperature-controlled electrode, a permanent lesion is created in the thalamus that helps to stop tremor without disrupting sensory or motor control.
