Former Wash Gov Booth Gardner Dies
OLYMPIA, Wash. Booth Gardner, a two-term Democratic governor who later in life spearheaded a campaign that made Washington the second state in the country to legalize assisted suicide for the terminally ill, has died after a long battle with Parkinson’s disease. He was 76.
Gardner died Friday at his Tacoma home of complications related to the disease, family spokesman Ron Dotzauer said Saturday.
“We’re very sad to lose my father, who had been struggling with a difficult disease for many years, but we are relieved to know that he’s at rest now and his fight is done,” said Gail Gant, Gardner’s daughter, in a statement.
He was the state’s 19th governor.
The millionaire heir to the Weyerhaeuser timber fortune led the state from 1985 to 1993 following terms as Pierce County executive, state senator and business school dean.
Since then, he had worked as a U.S. trade ambassador in Geneva, in youth sports and for a variety of philanthropic works. But his biggest political effort in his later years was his successful “Death with Dignity” campaign in 2008 that ultimately led to the passage of the controversial law that mirrored a law that had been in place in Oregon since 1997.
The law allows terminally ill adults with six months or less left to live to request a legal dose of medication from their doctors.
The Washington law took effect in March 2009, and since then more than 250 people have used it to obtain lethal doses of medication.
Medications For Restless Legs Syndrome
Dopaminergic agents. Levodopa is the best-studied treatment for RLS. It is inexpensive and has a fast onset of action . Levodopa is most appropriate for treatment of occasional, bothersome RLS symptoms rather than for daily use because of a common treatment complication termed augmentation, in which RLS severity increases over time. Signs of augmentation include onset of symptoms earlier in the day, shorter latency to symptoms at rest, and spread of symptoms to the arms and trunk. In severe cases of augmentation, RLS symptoms can persist throughout the day and despite physical activity. Augmentation is common, occurring in over 70% of patients treated with levodopa.
More appropriate for long-term treatment are the dopamine agonists ropinirole and pramipexole, which have recently been approved by the US Food and Drug Administration for treatment of RLS. Dopamine agonists are associated with a lower risk of augmentation relative to levodopa . Note that the onset of action is 90120 minutes, so the medication must be taken earlier in the evening than levodopa, before RLS symptoms appear.
In general, dopamine agonists are generally very well-tolerated treatments that provide excellent and sometimes dramatic, relief from RLS symptoms. Side effects include nausea, orthostatic hypotension, and hallucinations. Note that in this population, dopamine agonists can cause impulse control disorders such as compulsive gambling and hypersexuality, with a prevalence as high as 7%.

Booth Gardner, the charismatic Democrat who ousted Washingtons last Republican governor in 1984, launched the Basic Health Care program and later used his long personal battle with Parkinsons disease to spearhead the states death with dignity law, has died. Gardner was 76.
Gardner died March 15 of complications related to Parkinsons disease, a degenerative disorder of the central nervous system. Because Parkinsons itself is not fatal, the death with dignity law approved by voters in 2008 did not apply to Gardner.

Gardner, an heir to the Weyerhaueser timber fortune, was Pierce County Executive and little known elsewhere in the state when he entered the governors race in 1983. His campaign team adopted the signature slogan Booth Who?, and he went on to defeat now-U.S. Rep. Jim McDermott in the Democratic primary and upset incumbent Republican John Spellman in the general election.
Gardner disliked many of the public speaking aspects of campaigning and governing but was famous for his common touch both on the campaign trail and in the halls of Olympia. Although he had an MBA from Harvard University, the charismatic and popular Gardner liked to refer to his management style as MBWA Management By Walking Around.

Washington state had rejected a similar assisted-suicide initiative in 1991. But after a contentious campaign, in which Gardner contributed $470,000 of his own money toward the $4.9 million raised to support the measure, nearly 58% of voters approved the new law.
The law allows terminally ill adults with six months or less left to live to request a legal dose of medication from their doctors.
Gardner knew that he wouldnt qualify to use the law because his Parkinsons disease was incurable, not fatal. But his worsening condition made him a right-to-die advocate.

Treatment Of Restless Legs Syndrome
Address the underlying causes. Successful treatment of RLS relies on a thorough search for the etiology of the symptoms. Exclude or treat any possible secondary causes of RLS, including iron deficiency, which may itself be a marker for serious systemic disease such as gastrointestinal malignancy. Other common secondary causes include renal disease, neuropathy, and myelopathy.
Hyperparathyroidism, as described in the case report, is a rare cause of secondary RLS to our knowledge, this is the second reported case. As the case also illustrates, it is important to search for secondary causes, even in the setting of a positive family history, especially if there has been any significant worsening of symptoms.
Identify and discontinue exacerbating agents. A variety of medications can cause or worsen RLS, including tricyclic antidepressants, selective serotonin reuptake inhibitors, sedating antihistamines including diphenhydramine, dopamine receptor-blocking neuroleptics, and anti-emetics including metoclopramide. Alcohol, caffeine, and nicotine may also exacerbate symptoms.
Explore nonpharmacologic options. Good sleep hygiene includes regular bedtimes and wake times, and reserving the bedroom for sleep and sex only. Moderate, regular exercise may improve symptoms. Relaxation techniques such as hot baths, leg vibration, massage, and biofeedback may also be helpful.

I learned today that Governor Booth Gardner died yesterday. Governor Gardner was a source of inspiration, creative leader, compassionate man and friend to many. When diagnosed with Parkinsons, he used this to reach out to others in strength, support,and kindness. His concern for others proved him to be a man of grace and dignitiy. The Board, staff and supporters of the Northwest Parkinsons Foundation are deeply saddened by the passing of Governor Gardner. Booth was a leading voice for the Parkinsons community throughout the state and the entire country. As a long-time Board member, NWPF is indebted to him for his tremendous contributions toward raising general awareness of Parkinsons Disease and pushing for greater access to patient care and wellness. His legacy will endure through his work in establishing the Booth Gardner Parkinsons Care Center.
The Northwest Parkinsons Foundation sends its condolences to his family.
