Effects Of An Aquatic Physical Exercise Program On Ventilatory Parameters In People With Parkinsons Disease

Problems in the respiratory system are the main cause of death in Parkinsons disease . Ventilatory limitations can also be part of a vicious cycle involving physical-functional limitations and the patients perception of fatigue. The objective of this study was to analyze the effects of an aquatic physical exercise intervention program on ventilatory parameters, perception of fatigue, and gait capacity in participants with PD. This quasi-experimental study had a single group with repeated measures in four assessments, proposing an aquatic physical exercise intervention program. The inclusion criteria encompassed being in levels 1 to 4 on the Hoehn and Yahr scale and having a medical certificate for the activities. Assessments took place at 3-month intervals between themthe first period was the control, the second following the intervention, and the third period was the follow-up. The intervention had 25 biweekly sessions over 3 months. A total of 13 people participated in the intervention, without significant differences in the control period. Between the intervention assessments, they had statistically significant differences in MIP, MEP, FVC, Tiffeneau index, MVV, and fatigue. The study demonstrated that the aquatic physical exercise intervention was effective for ventilatory outcomes and fatigue in people with PD.

Whether genetic factors have been identified or note, the main pathological hallmark of PD is the selective degeneration of midbrain dopamine neurons in the substantia nigra . Many studies have consequently focused on how these neurons are formed and how to increase their lifespan. Studies using mouse genetic and in vitro models have demonstrated that the development and maintenance of mDA neurons are delicately regulated by genetic and signaling networks, including several transcription factors . During the development of mouse mDA neurons, unique sets of TFs are activated, including Engrailed-1/Engrailed-2, Foxa1/2, Lmx1a/b, Nurr1, Otx2, and Pitx3 . Recent single-cell RNA-seq analysis on developing human brains has confirmed their induction in human mDA neurons as well . Functional interactions between pairs of these TFs have been identified to ensure proper development of mDA neurons. Specification of mDA neurons progenitors depends on the interaction of signal molecules , growth factors , and a series of key TFs including En1/2, Otx2, Lmx1a/b, and Foxa1/2 . The subsequent differentiation and maturation of mDA neurons requires the expression of Nurr1 and Pitx3, which control the expression of several marker genes for mature mDA neurons, such as TH , DAT and VMAT2 , AADC , DRD2 , or ALDH1A1 .

Studies outlined above underline the role of TFs in development and also suggest the potential for use in cell replacement therapy for PD. Forced expression of Lmx1a or various combinations of TFs is also able to induce differentiation of human pluripotent stem cells into mDA neurons in vitro . In addition, several findings indicate that NURR1 alone or in combination with other TFs, such as FOXA2, NGN2, ASCL2, and PITX3, induces iDA neurons from hPSCs or human astrocytes or fibroblasts . More recently, several groups have reported direct cell fate conversion in vivo triggered by TFs with the overexpression of Ascl1, Lmx1a, Neurod1, and miRNA-218 in mouse brain directly converting astrocytes to mDA neurons that when transplanted, result in marked improvement in spontaneous motor behavior deficits in 6-OHDA-induced PD model mice .

The epigenetic changes linked to Parkinson’s diseasea nervous system disorder that afflicts nearly 1 million Americansare different in men and women, according to a new Rutgers study published in npj Parkinson’s Disease.

In a postmortem analysis of brain neurons, researchers compared samples from 50 people who died with Parkinson’s and 50 who had no sign of the disease. They found more than 200 genes with different epigenetic marks in diseased and healthy brainsbut the affected genes were almost entirely different in men and women.

“You could illustrate the male-female divide with two circles depicting genes with different epigenetic markings in Parkinson’s disease, one for men and one for women, and the overlap between the circles would only contain five genes,” said Alison Bernstein, senior author and assistant professor of pharmacology and toxicology at Ernest Mario School of Pharmacy.

“And we’ve found this every time we’ve looked at males and females separately, regardless of whether we’ve been looking at humans or mice or toxicology models. What we call Parkinson’s disease, singular, is probably Parkinson’s diseases, plural.”

“Some of the genes we found had already been implicated in other studies, but many of them were brand new, so this study opens up many avenues for further investigation of how these other genes are connected to Parkinson’s,” Bernstein said.
