While drinking caffeine each day does not appear to help improve sleepiness among people with Parkinsons disease, it may have a benefit in controlling movement, according to new research published in the August 1, 2012, online issue of Neurology®, the medical journal of the American Academy of Neurology.
The Effect Of Prolonged Caffeine Abstinence On Attention Is Limited
With the exception of PD patients on the CRT, testing participants following a 1-week caffeine abstention was not associated with impaired performance compared to baseline testing whilst on habitual caffeine consumption. Rather, as expected, testing participants after abstinence was associated with improved performance, mostly likely due to practice effects. Most studies to date were conducted during the acute withdrawal period and our results do not detract from the caffeine withdrawal reversal hypothesis . Instead, these results confirm an abstention period of a week is long enough to pass beyond withdrawal, avoiding the associated negative attentional effects. Any future research examining the acute cognitive effects of caffeine should ensure a similar duration of abstention as a minimum. Future research is required to ascertain whether the effects of caffeine abstention found here are genuinely due to the removing caffeine from daily life or are driven by practice effects.

Mechanisms Remain A Mystery
Commenting on the findings for Medscape Medical News, Clemens Scherzer, MD, professor of neurology, Harvard Medical School in Boston, said, we have known for some time that coffee drinking and smoking are strongly associated with reduced risk of developing PD, although the exact mechanisms and the exact chemicals at play remain mysterious.
Overall, the data did not provide a clear answer on aspirins possible effects, said Scherzer, who is also director, American Parkinson Disease Association Center for Advanced Research at Brigham & Womens Hospital, attributing the uncertainty to the type of analysis, confounding variables, and the studys retrospective design.
Instead, he added, the results suggest that this might be a clue worthy of further investigation and clarification.
While noting the studys observations are exploratory and preliminary, Scherzer, who was not involved in the research, noted coffee lovers, however, can feel good about this study. Risk is reduced and Parkinsons disease might start later in people who drink coffee.
Also commenting on the research, Andrew Siderowf, MD, professor of neurology, Perelman School of Medicine, University of Pennsylvania, Philadelphia, who was not involved in the study, urged caution in interpreting the findings.
The researchers, Scherzer, and Siderowf report no relevant disclosures.

Coffee Caffeine And Risk Of Parkinsons Disease
A large number of epidemiological studies report an inverse, dose-responsive relationship between coffee/caffeine consumption and the risk of developing PD. Coffee consumption appears to reduce or delay the development of PD and caffeine is most likely the causal factor. In women, however, the interaction between caffeine and hormonal therapy still needs further clarification.
As early as 1968, a study reported a higher percentage of non-coffee consumers in the control compared to the affected group47. Subsequent studies performed in Spain48, Sweden49 and Germany50 found an inverse relationship between coffee consumption and PD, and a lower coffee consumption before disease onset.
The first prospective study on 8,004 Japanese American men living in Hawaii , carried out over a median duration of 27 years, reported an inverse association between PD incidence with a five-fold reduced risk for those drinking more than 4 cups of coffee per day when compared to non-consumers. The same inverse relationship was shown for caffeine from non-coffee sources51. A smaller case control study published in 2014 suggested only a weak inverse association between coffee intake and the risk of PD52.
A number of meta-analyses and reviews have been undertaken which together support the view that coffee consumption is associated with a reduced risk of Parkinsonâs Disease.
Caffeine From Two To Four Daily Cups Of Coffee May Reduce Parkinsons Disease Symptoms
Should doctors start prescribing coffee?
In the latest study to tie coffee to health benefits, new research finds a daily caffeine boost may ease symptoms of Parkinsons disease in sufferers.
Parkinsons is a degenerative brain disease that causes tremors, difficulty with walking, movement and coordination. The condition most commonly develops in adults over 50, and occurs when nerve cells in the brain that make the muscle-controlling neurotransmitter dopamine are slowly destroyed, leading to loss in muscle function. The cause of the disease remains unknown.
The study, published August 1 in the online issue of Neurology, tracked 61 people with Parkinsons who were exhibiting symptoms of daytime sleepiness. Participants were assigned to receive either a placebo pill twice per day or a 100 milligram-caffeine pill twice per day for three weeks. After three weeks, those in the caffeine group were given a 200 milligram caffeine pill twice per day, about the caffeine equivalent found in two to four cups of coffee.
Six weeks into the study, people taking the caffeine pills averaged a five-point improvement on a ratings scale of Parkinsons symptom severity compared to those who took placebos. Researchers called this a modest improvement, but one that might provide benefits to people with the disease.
Oddly enough, caffeine did not appear to help improve the patients daytime sleepiness problems.
