  • The instructor should have expertise in PD. APDA offers a training for fitness professionals which helps them better understand how to create exercise programs best suited for people with PD.
  • The ratio of class members to instructors should be low enough to maintain safety.
  • The class should be adaptable for people of different abilities and mobility. Many classes will offer seated options or other modifications.

It may take a few tries to find the types of exercise that suits you best so dont give up! Feel free to try different classes in your area to see what types of exercise appeal to you. It can also be motivating to take classes together with a friend or family member. You can hold each other accountable, and also have some fun together. As you find classes you enjoy, you can incorporate those modalities into your exercise schedule. There are many different forms of exercise, but what type is right for you? Every individual is different! Its important to remember that although a combination of aerobic, resistance and balance exercises have the best overall effect, you may need to modify each element to your suit your unique circumstance.

Aerobic exercise

Aerobic exercise is described as continual movement to assist in the improvement of cardiorespiratory function. This includes walking, cycling, swimming and even dancing! Exercising to music specifically has seen some fantastic results in managing Parkinsons symptoms. Data Sources And Search Strategy

Five databases were used during article selection process from February 2015 to September 2015. An electronic database search for relevant Randomized controlled trials which examined physiotherapy techniques used to treat, BD and PI among people with PD of any duration and published in international medical journals in the English language from 2005 to June 2015was conducted. We searched articles using keywords of RCTs, Parkinsons disease, physiotherapy, postural instability, balance dysfunction, Exercise, equilibrium, postural control, and rehabilitation.

The relevance of the reviewed studies was checked based on their topic, objectives, and methodology. Preliminary assessments have been made and some articles were excluded at the first step just by looking at the topic. On the second step, abstracts have been seen and were excluded if they did not match to the current study objectives. For the rest, the whole content of the articles was accessed and selected based on the independent and dependent variables under review.

Molecular Mechanisms For The Benefits Of Exercise

Animal studies suggest several different mechanisms in which exercise leads to neuroprotective effects and promotes neuroplasticity. Neuroplasticity, defined as the ability of neuronal circuits to make structural and functional adaptive changes, can be induced by exercise and effect the nigrostriatal and related motor circuits that are involved in the pathophysiology of PD.3,4 Endurance exercises promote neurogenesis and neuroprotection in animals.9 Several studies have shown that exercise exerts neuroprotective effects against dopaminergic neurotoxins 6-hydroxydopamine and 1-methyl,4-phenyl,1,2,3,6-tetrahydropyridine .10,11 In a transgenic mouse model of diffuse Lewy body disease, mice that exercised for 3 months had significantly less -synuclein aggregation than in the brains of sedentary mice.12 These findings suggest that in animal models of PD, exercise has neuroprotective effects.

Exercise has been shown to increase dendritic length and complexity and dendritic spine density within the hippocampus. Exercise increases expression of genes associated with neuroplasticity and downregulates genes linked to oxidative stress.3 In a study involving a transgenic mouse model of Alzheimers disease , 5 months of exercise significantly reduced brain amyloid concentrations.3,14 These changes are associated with improved performance in spatial memory and object recognition.3

Outcome Measures And Test Procedure

The primary outcome is balance, which will be assessed by the Berg Balance Scale , postural stress test , push and release test and Mini BESTest . The secondary outcome is gait, which will be evaluated by the timed up and go test and by the freezing of gait, using the Freezing of Gait Questionnaire . Independence in activities of daily living , and motor performance will be assessed by the unified Parkinsons disease rating scale . Falls and fear of falling will be evaluated during a variety of everyday activities and measured by the Falls Efficacy Scale-International .

The number of falls will be registered by the physiotherapist at the hospital and by the patient at home. A weekly follow-up of falling will be conducted.

Patient And Public Involvement

To ensure that the design and content of the tested trial would be meaningful and important to key potential stakeholdersi.e., people living with Parkinsons and those close to thempatient and public involvement activities were carried out during the design phase with the support of the Parkinsons UK Research Involvement team. First, a survey was disseminated to the Parkinsons UK Research Support Network. Of the 329 people who responded to the survey, 282 were living with Parkinsons 41 identified as carers, partners, family members, or friends and 6 were bereaved carers, partners, family members, or friends. The survey sought to understand if the aims of the research were clear, whether or not this area of research is seen as important by people affected by Parkinsons, and how likely people would be to take part or recommend someone they know take part in the study. Responses revealed 97.9% of people living with Parkinsons who took part in the survey found the aims of the research very or somewhat clear, 98.5% found the study to be very or somewhat important, and 79.4% indicated that they would be very or somewhat likely to participate, which supported the feasibility of such a trial. The survey also explored barriers to participation, the feasibility and selection of outcome measures, and ideas for capturing the experience of dancing .

Gait Symmetry And Dual Task Performance

The role of dance practice in reducing PD motor symptoms has been previously reviewed showing the beneficial effects of dance on motor symptoms , balance , and walking velocity . Hashimoto et al. reported effects on both motor and cognitive functions . Importantly, decline in motor performance and cognitive impairment have the greatest influence on the quality of life of people with PD .

Further, gait initiation can be modulated by the presentation of emotional visual stimuli in patients with or without freezing of gait , as evidenced by longer reaction times and shorter steps in response to incongruent/unpleasant condition . This evidence may suggest an interplay between gait performance and emotional processing, since the basal ganglia act as gatekeepers in both approach motivation and avoidance responses . This interplay may be relevant to this study, as we reported affective responses paralleled by electrodermal activity, which significantly differed between the two conditions. In particular, after Dance for PD and differently from matched-intensity exercise, participants showed higher emotional arousal , affective modulation , gait symmetry, and dual task performance.

Exercise is often the primary reason people first try a dance for Parkinsons class. Once they experience the music, the movement, the magic of dance firsthand, they fall in love with it. Then they discover it is so much more than just a great workout.

Dancers with Parkinsons talk about experiencing an easing of symptoms during class. Margie Dahlin explains,

I always start class with a tremor, and by the time class is over, I dont have a tremor at all. I move differently, I carry myself differentlyI can sense the change. You go with the flow and become so wrapped up. It is such a mindful experience. I feel like something has been lifted off my shoulders and I feel like a new person.

Dancing provides an arena to feel like an integrated body working as a whole instead of feeling like a patient. In dance class we welcome both the struggling parts and thriving parts of the body, as well as the awareness of how things can change unexpectedly day to day. Embracing and honoring your dancer body can be liberating and fortifyingit is something gained among so many things lost when dealing with Parkinsons.

Breathing compassion into the struggling parts, breathing gratitude into the thriving parts and the ability to see all those parts as an integrated, whole body is a practice that participants can carry with them every day.

What Type Of Dance Is Best For Pd

As a fitness instructor, my answer is always going to be that the best type of dance for PD is the type that you will actually practice consistently. It won’t benefit you if you don’t do it. Most of the studies mentioned above found that the benefits of dance for PD patients came from doing it regularly over a period of weeks.

Every person with Parkinson’s is different. Not all PD sufferers have the same exact set of symptoms, nor will they respond identically to a certain form of dance. Here are a few types of dance that have been shown to have specific value for people with PD.

  • Tango. One small research study suggested that tango might be a strategy for improving functional mobility deficits. Tango can involve learning specific ways of moving that might improve gait and balance.5
  • Dance for PD. These are specific classes taught by professional dancers. These classes include modern dance, ballet, tap, folk and social dancing. Emphasis is on experiencing “the joys and benefits of dance while creatively addressing symptom-specific concerns related to balance, cognition, motor skill, depression and physical confidence.”
  • Zumba Gold.Zumba is a dance fitness program that combines these elements of fitness: cardio, muscle conditioning, balance and flexibility, choreographed to Latin and world music. The Gold version of Zumba is a lower-intensity, low impact version that emphasizes balance and coordination, along with social interacton.
